1. The Site Is Informational — Not Medical Advice
The content on this Site is provided for general informational and educational purposes only and is not a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of a qualified health provider with any questions about a medical or mental health condition. Reading this Site or contacting us through it does not create a provider-patient relationship.

2. No Emergency or Crisis Services
The Site and its forms are not monitored in real time and must not be used for emergencies or urgent clinical needs. Do not use the Site to communicate time-sensitive or emergency information.
3. Do Not Submit Health Information
Please do not submit protected health information (diagnoses, symptoms, medications, treatment history) through the Site’s forms or messages. Any necessary health information will be collected securely and privately through appropriate channels.
4. Appointment Requests
Submitting a form or requesting an appointment through the Site is a request only and does not confirm an appointment. An appointment is confirmed only when our staff contacts you to schedule it.
5. Use of the Site
You agree to use the Site lawfully and not to:
- interfere with its operation or security
- attempt to access areas you are not authorized to access
- upload harmful code
- use the Site to harass, harm, or impersonate others

8. Disclaimers & Limitation of Liability
The Site is provided ‘as is’ without warranties of any kind, to the fullest extent permitted by law. We do not warrant that the Site will be error-free or uninterrupted, or that information is complete or current.
To the fullest extent permitted by law, MMH and its owners, members, officers, employees, and agents will not be liable for any indirect, incidental, special, consequential, or punitive damages, or for lost profits, data, or goodwill, arising out of or relating to your use of, or inability to use, the Site, even if advised of the possibility of such damages. MMH’s total aggregate liability for all claims arising out of or relating to the Site or these Terms will not exceed one hundred U.S. dollars ($100). Nothing in these Terms limits any liability that may not be limited under applicable law, and nothing in these Terms alters the professional obligations owed to patients in connection with clinical care actually provided.
You agree to indemnify and hold harmless MMH and its owners, officers, employees, and agents from and against claims, damages, and expenses (including reasonable attorneys’ fees) arising out of your violation of these Terms or your misuse of the Site.
9. Privacy
Your use of the Site is also governed by our Privacy Policy, which is incorporated into these Terms by reference.
10. Changes & Governing Law
We may update these Terms from time to time; the ‘effective date’ shows the latest version.
These Terms are governed by the laws of the State of Florida, without regard to conflict-of-law principles. Any dispute arising out of or relating to the Site or these Terms that is not governed by another written agreement between the parties shall be brought exclusively in the state or federal courts located in Miami-Dade County, Florida [CONFIRM venue: Miami-Dade (Hialeah office) or Broward (Plantation office)], and each party consents to personal jurisdiction and venue there.
