This Privacy Policy explains how Managed Mental Health ("MMH," "we," "us," or "our") collects, uses, and protects information when you visit our website at www.mmhfla.com (the "Site"). It applies to information collected through the Site only. It is separate from the privacy protections that apply to your protected health information as a patient, which are described in our Notice of Privacy Practices.
1. Important: Do Not Share Health Details on This Site
Our website forms are for general contact and scheduling requests only. Please do not include private health information — such as diagnoses, symptoms, medications, or treatment history — in any website form or message. We will gather any necessary health information securely and privately when we speak with you or during your visit.

2. Information We Collect
Information you give us: When you fill out a contact, appointment, subscribe, or ‘Find Your Starting Point’ form, we collect the information you choose to provide — typically your name, phone number, email address, preferred location and language, general (non-clinical) reason for reaching out, and your communication preferences.
Information collected automatically: Like most websites, we may automatically collect limited technical information such as your IP address, browser type, device, pages visited, and how you arrived at the Site, through cookies and similar technologies and analytics tools. We do not use advertising or social-media tracking pixels (such as the Meta/Facebook pixel or Google Ads tags) on the Site, and we do not use session-recording tools that capture the contents of forms.
3. How We Use Your Information
- To respond to your inquiry, schedule or confirm appointments, and provide the information or services you request.
- To send you communications you have opted in to receive — such as appointment information, newsletters, wellness tips, and promotional or marketing messages — by email and/or text message.
- To improve our Site and understand how visitors use it.
- To meet legal, regulatory, and safety obligations.
We do not sell your personal information.
4. Email & Text Message Communications (Opt-In)
If you check the box agreeing to receive communications, you consent to receive emails and/or text messages from MMH, which may include promotional or marketing content. Message and data rates may apply. Consent is not a condition of receiving care. You may opt out at any time by replying STOP to a text message or clicking ‘unsubscribe’ in any email. We honor opt-out requests promptly. Your consent to receive marketing messages is collected through a separate, optional checkbox that is not pre-checked, and applies to the phone number or email address you provide. You may also opt out by any other reasonable method, including by contacting us as described in Section 11.
5. Cookies & Analytics
We use cookies and similar technologies to operate the Site, remember preferences, and understand usage. You can usually set your browser to refuse cookies, though some features may not work as well.
6. How We Share Information
We share information only as needed to operate, and only with parties who are obligated to protect it:
- Service providers that help us run the Site and our communications (for example, our website host, form provider, and email/text platform), under agreements that require them to safeguard the information — including a HIPAA Business Associate Agreement with each vendor that receives or stores identifiable information submitted through the Site by prospective or current patients (for example, our website form and scheduling providers and our email/text messaging platform).
- When required by law, or to protect the safety, rights, or property of our patients, staff, or others.
We do not sell or rent your personal information to third parties for their own marketing.
7. How We Protect Information
We use reasonable administrative, technical, and physical safeguards to protect information submitted through the Site, including encryption in transit (HTTPS). No method of transmission over the internet is 100% secure, which is another reason we ask you not to send health details through the Site.
8. Your Choices & Rights
- You can opt out of marketing emails and texts at any time (see Section 4).
- You can ask us what information we have from your Site submissions, and request correction or deletion, subject to legal requirements. We will respond to verifiable requests within a reasonable time.
Florida residents: We do not sell your personal information—including any information revealing your physical or mental health—and we do not use information submitted through the Site for targeted advertising. You may request access to, correction of, or deletion of information you have submitted through the Site, and we will honor verifiable requests within 45 days unless the law requires us to retain the information. You will not be denied care or otherwise penalized for exercising these choices. Health information we maintain about you as a patient is protected separately under HIPAA and our Notice of Privacy Practices, and requests concerning patient records are handled under those rules. If a data breach affects your personal information, we will provide notice as required by the Florida Information Protection Act, section 501.171, Florida Statutes.

9. Children’s Privacy
The Site is intended for adults. We do not knowingly collect personal information from children under 13 through the Site without a parent or guardian. Parents or guardians may submit information on behalf of a minor when seeking care for them. If you believe a child has provided information through the Site, please contact us so we can address it.
10. Links & Changes
Our Site may link to other websites (such as directories or social media); we are not responsible for their privacy practices. We may update this Policy from time to time; the ‘effective date’ above shows the latest version, and material changes will be posted on this page.
