Managed Mental Health complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, sex consistent with applicable law, age, or disability.
Why this notice is required
Section 1557 of the Affordable Care Act prohibits discrimination in health programs and activities that receive federal financial assistance. Because Managed Mental Health participates in Medicare and Medicaid, it is subject to that law and to the U.S. Department of Health and Human Services regulation implementing it (45 CFR Part 92).
That regulation requires two separate notices — a notice of nondiscrimination (45 CFR § 92.10) and a notice of the availability of free language assistance services and auxiliary aids and services (45 CFR § 92.11) — and requires both to be posted at a conspicuous location on the practice's website. This page carries both.
Our nondiscrimination commitment
Managed Mental Health does not unlawfully discriminate in its health programs or activities on the basis of race, color, national origin (including limited English proficiency and primary language), sex consistent with applicable law, age, or disability. This applies to every part of our practice — scheduling an appointment, being accepted as a patient, the care you receive from our therapists and psychiatric providers, telehealth visits, and billing.
Managed Mental Health also does not discriminate against an individual solely because the person is covered by Medicare, Medicaid, Medicaid managed care, Medicare Advantage, commercial insurance, or is self-pay. Legitimate, nondiscriminatory requirements related to eligibility, network status, authorization, benefits, finances, clinical appropriateness, or capacity may still apply.
Free services to help you communicate with us
Managed Mental Health provides the following free of charge, when required by applicable law, to people who need them. You will not be required to supply your own qualified interpreter when MMH is legally responsible for providing one.
- Free language assistance services to people whose primary language is not English, including qualified interpreters and information written in other languages
- Free aids and services to people with disabilities to help them communicate effectively with us, including qualified sign language interpreters
- Written information in other formats, such as large print, audio, accessible electronic formats, and other formats
- Reasonable modifications to our policies and procedures when needed to give a person with a disability equal access to our care
How to ask for these services
If you need any of the services listed above, please tell us — by phone at our Hialeah office, (786) 233-6721, or our Plantation office, (954) 666-5942, or by email at [email protected]. Please ask as far ahead of your appointment as you can, so we have time to arrange an interpreter, but tell us whenever you realize you need help and we will do our best. You do not need to use specific legal words such as "ADA" or "accommodation" — just tell us what you need.

About the notices above
The fifteen languages above are Florida's most common languages spoken by individuals with limited English proficiency, as identified by the U.S. Department of Health and Human Services, Office for Civil Rights — the only state-by-state list OCR has published (August 2016). Each notice carries both of our office phone numbers and 711, the FCC-mandated nationwide relay number, for TTY access.
These are HHS's own official translations, reproduced as published, including the source documents' own minor imperfections (for example, small grammatical artifacts introduced by inserting a real phone number into a translated template) rather than corrected by a non-qualified translator. Chinese is published in both Traditional and Simplified script to serve more readers.
This availability notice also appears in our Notice of Privacy Practices, satisfying 45 CFR § 92.11(c)(5)'s requirement that it be included in specified patient-facing documents. If you believe we have discriminated against you
If you believe that Managed Mental Health has failed to provide any of the services described above, or has discriminated against you on the basis of race, color, national origin, sex consistent with applicable law, age, or disability, you may submit a complaint to MMH Administration, the Compliance Officer, or the Section 1557 Coordinator, as applicable. You may also file a civil rights complaint directly with the U.S. Department of Health and Human Services, Office for Civil Rights, or another governmental agency with jurisdiction.
No retaliation. MMH prohibits retaliation, intimidation, coercion, threats, interference, or adverse treatment because a person requested assistance, filed or threatened to file a complaint, contacted the Office for Civil Rights, participated in an investigation, or helped another person exercise a protected right. Filing a complaint will not affect your care.
How to file a complaint with the federal government
You can file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, electronically through the Office for Civil Rights Complaint Portal at ocrportal.hhs.gov, or by mail or phone at: U.S. Department of Health and Human Services, 200 Independence Avenue, S.W., Washington, D.C. 20201; telephone (800) 368-1019; TDD (800) 537-7697. This notice must also be posted in our offices
Publishing this web page does not satisfy every obligation under this notice. Under 45 CFR § 92.10, this notice must also be provided on an annual basis to patients and applicants, provided upon request, and posted in a clear and prominent physical location at both the Hialeah and Plantation offices in no smaller than 20-point sans serif font, where people seeking care can reasonably be expected to see it. Section 92.11's notice of availability carries its own placement requirements, including in specified patient communications.
